Youth Athletic Development Registration

Please complete the registration form below for your junior. One form must be completed for each junior registering. If you have an account with Cedardale we will process your registration with your current card on file. If you do not have an account with Cedardale we will contact you shortly by phone to gather your form of payment and complete your registration. Fees for the Youth Development program are billed monthly. Participation and monthly billing will continue unless Cedardale is notified by the 25th of the month for cancellation for the next month.

We will contact you if we can not accommodate your registration. Cedardale reserves the right to use photos taken during lessons or programs for promotional purposes.
